Players who have not been treated by Campbell Orthodontics before must have a parent fill out the paperwork below to bring to the appointment. Unfortunately, players who currently have braces will not be eligible for these custom mouth guards. However, Campbell Orthodontics does have orthodontic mouth guards that they can give if the individual does have braces.
